[LCC] W5L special event - online form to request access to scheduling tool

We've got an online form set up to request a login for the W5L 
scheduling tool, which will enable you to reserve your own time slots to 
operate during the special event.  Remember, this event will allow you 
to operate from anywhere in Louisiana - there is not a central location 
that is hosting this event.
